Applicants may submit their completed application using either of the following methods:

By Mail: download an application form HERE and mail it to P.O. Box 65, Miller City, Ohio 45864
Please submit only one application per proposed project or funding request.

Application Review & Award Decisions

All applications will be reviewed by Connections for Autism and Other Disabilities. All grant award decisions are made in the sole discretion of the organization and are final. Submission of an application does not guarantee funding, and the organization is not obligated to award all available grant funds. Applicants may be contacted for additional information, documentation, or clarification during the review process and may be invited to participate in an interview.

Connections for Autism and Other Disabilities reserves the right to approve, deny, or partially fund any application and may award an amount different from the amount requested by the applicant. The organization makes no representations regarding the tax treatment of grant funds. A submitted application does not create any contractual right to receive funding.
